There are study abroad options where classes are conducted in English, but in order to get around and appreciate the country and the culture you really need to speak Italian. This is one of the European countries where all foreign films are dubbed (not subtitled) and outside of the cities (and often in the cities), you will not find that English is easily understood. So, take those Italian classes in school or find a way to cram but have a basic ability in the language before you go!
