In order to qualify, students must meet the following criteria:
  1. Must reside in Middletown or Monroe, Ohio.
  2. Must be a first-generation college student.
  3. Must be a U.S. citizen or legal permanent resident.
  4. This award is for U.S. students.
  5. Must have a grade point average of 2.5 or higher.

Description: This scholarship is available for high school seniors graduating in Middletown or Monroe, Ohio. Students must be the first member of their families to pursue a college education.

Additional Information: This scholarship is provided annually in honor of Norman and Pat Hayes, residents of Middletown who demonstrated a tireless commitment to education. After moving to Middletown, Norman Hayes became a teacher in Middletown and Monroe schools, while Pat was first employed as an education director at a local church. Norman's public school career would continue until he retired as superintendent, at which time he continued to work for local educational causes by serving as the executive director of the Middletown Community Foundation.

Award Amount: $1,000 annually, renewable for up to three additional years so long as the student maintains a grade point average of 2.0 or higher.  The total dollars awarded for this scholarship is $1,000.

To be considered for this award, students must submit an online application form, including the personal information section, family information, and financial information. In addition, the student must also submit a copy of his/her most recent high school transcripts including SAT/ACT scores, and a resume detailing work experience and/or community service participation. All applications and supporting documents must be submitted online by the deadline date.
