Number of Awards & Eligibility: Three awards offered annually.
In order to qualify, students must meet the following criteria:
  1. This award is for U.S. students.
  2. Must be enrolled in a physical therapy program.
  3. Must be a resident of Hawaii.
  4. Must be a U.S. citizen.
  5. Must attend an accredited four-year college or university as a full-time student (12 credits per semester for undergraduate students; 9 credits per semester for graduate students).
  6. Must demonstrate academic achievement with a grade point average of 2.7 or higher.
  7. Must be a college junior, senior or a graduate student.

Description: This scholarship is for Hawaii college juniors and seniors and graduate students who are enrolled in a physical therapy program. Students must have a grade point average of 2.7 or higher.

Additional Information: This scholarship was established from the Trust of Paulina Sorg for students studying Nursing or Physical Therapy. Ms. Sorg formerly worked at the Department of Health in the Bureau of Crippled Children and Maternal and Child Health. Her dedication to the patients she served in life continues on through this scholarship. All applicants must demonstrate financial need and good moral character. Recipients will be notified after the scholarship selection committees meet, which could be any time between May and August.

Award Amount: Approximately $1,000 annually, non-renewable. Students may reapply each year.  The total dollars awarded for this scholarship is $3,000.

Online application forms are available on the Hawai'i Community Foundation website. The student should visit the provider's website for detailed instructions on how to complete the online application. In addition to the online application form, the student must submit the following supporting materials: the printed confirmation page that is generated after applying online; a typed, double-spaced personal statement of two to three pages answering the questions listed online; an official copy of his/her most recent transcript; and a copy of his/her SAR (Student Aid Report). The SAR is obtained by filing the FAFSA (Free Application for Federal Student Aid). The FAFSA is available online, at high school guidance offices, and at college financial aid offices. Note: Students WILL NOT be able to submit their application until all required supporting documents are electronically attached to the application. The application must be submitted by 4 p.m. HST on the deadline date.
