Number of Awards & Eligibility: One award offered annually.
In order to qualify, students must meet the following criteria:
  1. This award is for U.S. students.
  2. Must be female.
  3. Must be pursuing an M.D. degree.
  4. Must be of Polish descent.
  5. Must have a grade point average of 3.0 or higher.
  6. First preference is given to residents of Massachusetts. Qualified residents of New England are considered if no first preference candidates apply.
  7. Must be a U.S. citizen or permanent resident of the United States.
  8. Must be entering her first, second, or third year of medical school.

Description: This scholarship is available for U.S. female students who are of Polish descent and enrolled or planning to enroll in medical school. Students must have a grade point average of 3.0 or higher. First preference is given to residents of the state of Massachusetts; qualified residents of New England are considered if no candidates from first preference apply.

Additional Information: Selection is based on the following criteria: academic excellence, the applicant's academic achievements, interests, motivation, interest in Polish subjects, and involvement in the Polish American community. Financial need is taken into consideration. Scholarship notifications will be made in writing on or around May 29.

Award Amount: $3,500 annually, non-renewable.  The total dollars awarded for this scholarship is $3,500.

Applications are available online from the Kosciuszko Foundation website. In addition to a completed application, each student must submit the following: a $35 non-refundable application fee; a statement of purpose (discussing her academic goals, major, and planned specialization); a passport-size photo; official transcripts for the last three full years of school work through summer 2018; two letters of academic reference submitted on letterhead and in sealed envelopes; a copy of her letter of her letter of acceptance to medical school; and proof of Polish ancestry (using birth certificates, passports, naturalization papers and other official documents to show the line of descent from the ancestor who was born in Poland to the student). If documentation is not available, a letter from a prominent member of the community verifying Polish ancestry will be accepted. (Letters from relatives are not accepted.) Application materials should not be stapled. Application packets will not be accepted via fax. Certain components of the application packet must be uploaded to the online application, and other components may be emailed; the student should visit the Kosciuszko Foundation website for more details. The Kosciuszko Foundation cannot confirm receipt of materials by phone. All applications and supporting documents must be received no later than the deadline date.
