Number of Awards & Eligibility: One award offered annually.
In order to qualify, students must meet the following criteria:
  1. Must reside in Middletown, Ohio.
  2. Must plan to major in math or math education.
  3. Must be a U.S. citizen or legal permanent resident.
  4. This award is for U.S. students.
  5. Must be a high school senior at the time of application.
  6. Must have a grade point average of 3.5 or higher.

In order to qualify, students must meet the following criteria:

Description: This award is available for high school seniors in Middletown Ohio who are planning to major in mathematics or math education. Applicants must have a grade point average of 3.5 or higher.

Application requirements for the Mildred T. Fisher Scholarship are:

Additional Information: Mildred Fisher was a long time teacher of Mathematics at Roosevelt Junior High School. For more than thirty years her algebra students consistently ranked in first, second, third places in the Ohio Scholarship Tests. In her honor, one award will be provided each year to a Middletown, Ohio high school senior who plans to major in mathematics or mathematics education.

Award Amount: $140 annually, non-renewable.  The total dollars awarded for this scholarship is $140.

To be considered for this award, students must submit an online application form, including the personal information section, family information, and financial information. In addition, the student must also submit a copy of his/her most recent high school transcripts including SAT/ACT scores, and a resume detailing work experience and/or community service participation. All applications and supporting documents must be submitted online by the deadline date.
