Number of Awards & Eligibility: Two awards are provided annually. One scholarship will be granted for each of the deadline dates.
In order to qualify, students must meet the following criteria:
  1. This award is for U.S. students.
  2. Must be an undergraduate or graduate student.
  3. Must have a grade point average of 3.7 or higher.

Application requirements for the iVein® Health & Wellness Scholarship are:

Additional Information: The iVein® Health & Wellness Scholarship is provided biannually to recognize the importance - and challenges - associated with maintaining a healthy lifestyle in college and throughout life. Judges will look for specific examples from the applicant's personal experiences, as well as other details that provide evidence of understanding the importance of lifestyle on good health. The judges will also consider all applicants' academic records and grade point averages as evidence of a strong commitment to academic excellence. One winner will be contacted within three weeks of each deadline date.

Award Amount: $2,500 annually, non-renewable. All awards are provided on a one-time basis and previous recipients are not eligible to reapply. Funds will be paid directly to the recipients' schools.  The total dollars awarded for this scholarship is $5,000.

Application instructions and additional information about this scholarship can be found online at the Intermountain Vein Center website. To be considered for an award, students must write an essay of between 800 and 1,000 words in length. Written submissions must address strategies for sustaining a healthy lifestyle while attending college, and should include personal experiences that demonstrate a strong understanding of the subject. When completed, students must save the essay in a typical word processor format such as Microsoft Word (.doc, .docx), Adobe PDF (.pdf), or Google document. The filename must include the applicant's last name. In addition to the essay, students must also submit proof of a current eligible grade point average such as a transcript or other record of completed coursework. Students who are enrolled in a pass/fail program, such as medical school, may provide proof of their academic progress in lieu of a grade point average. All essays must be submitted by email and must be received no later than the deadline date. Any late or incomplete applications will be discarded.
