In order to qualify, students must meet the following criteria:
  1. This award is for U.S. students.
  2. Must be a U.S. citizen or permanent resident.
  3. Must be an undergraduate or graduate student.
  4. Must be diagnosed with cystic fibrosis.

Description: This scholarship is open to U.S. undergraduate and graduate students who have been diagnosed with cystic fibrosis. Students are selected based on academic achievements and adherence to daily cystic fibrosis therapy.

Additional Information: The Sacks for CF Scholarship is related to quarterback sacks made during the NFL season. For every sack recorded during NFL Monday Night Football games, the Sacks for CF Scholarship Program receives a donation from a Boomer Esiason Foundation corporate partner. Scholarships are awarded based on applicants' academic achievements and adherence to daily cystic fibrosis therapy. The names of scholarship winners will be announced on Super Bowl Sunday.

Award Amount: $3,000 to $10,000 annually, non-renewable.  The total dollars awarded for this scholarship is $6,500.

Applications are available on the Boomer Esiason Foundation website. As part of the online application, the student must provide the following materials: an essay of no more than 300 words discussing the importance of compliance to cystic fibrosis therapies and what he/she practices daily to stay healthy; an essay of no more than 300 words discussing his/her post-graduation goals; a recent photograph; a letter from his/her doctor confirming diagnosis of cystic fibrosis; a transcript (high school, college, and/or graduate school); a letter of acceptance from his/her institution; a detailed breakdown of tuition costs from academic institution; and W2 forms for both parents (as appropriate) or a tax return if self-employed. Applications must be submitted online by the deadline date.
